EarlyON COVID-19 Screening Form

EarlyON clients and their families – Please answer the following questions.  Your answers pertains to all names you have listed.  If you answer yes to any one of them your child/ren may not attend the program today. Please contact Centrepointe Childcare Services at 613-225-9678 to discuss any further action.

1 Step 1
2) Do you, your child(ren) or anyone in your/their household have any of the following symptoms:Symptoms should not be chronic or related to other know causes or conditions.
3) Do you, your child(ren) or any person in your/their household have any "two" of the following new or worstening symptoms?Symptoms should not be chronic or related to other know causes or conditions.

According to the Ministry of Health, if you answered yes to questions 2 and/or 3 for you or anyone in your household, it is highly likely that you/they have a COVID-19 infection.

•You must self-isolate immediately:

    • For at least 5 days from your symptom onset (day 0) and until your symptoms have been improving for 24 hours (or 48hours if gastrointestinal symptoms) whichever is longer in duration if you are:

         1. 12 years of age or older AND fully vaccinated.

         2. 11 years old or younger, regardless of your vaccination status

    • For 10 days from your symptom onset if you are:

         1. 12 years of age or older AND either partially vaccinated or unvaccinated.

         2. Immune compromised, regardless of your age

    •If your symptoms worsen, seek advice from Telehealth (1-866-797-0000) or your health care provider.

    •Notify your child care and/or workplace.

    **Please note:  If you/your child is exhibiting only one of the following new symptoms:

  •  runny/stuffy nose, 
  • sore throat, 
  • Conjunctivitis (pink eye)
  • Headache
  • Extreme tiredness, lack of energy
  • Muscle aches and pains
  • GI symptoms (i.e. vomiting or diarrhea)
  • It is less likely that you/your child has COVID-19 infection.
    You/your child must self-isolate until symptoms are improving, without the use of fever reducing medication, for at least 24 hours (48 hours for gastrointestinal symptoms).

    Your household members do not need to self-isolate.

    4) In the last 10 days have you, your child(ren), or anyone in your/their household travelled outside of Canada and been told a) to quarantine or b) not to attend school/child care?
    5) In the last 10 days, have you, your child(ren), or anyone in your/their household been identified as a close contact of someone who is confirmed as having a COVID-19 by your local health unit (or from the COVID Alert app if they have thier own phone)?
    6) Is anyone in your/their household waiting for a COVID-19 test result?
    7) Have you or anyone attending the session tested positive using a "Rapid Antigen" test in the last 5 days.
    8) Has a doctor, health care provider, or public health unit told you that you, your child(ren) or anyone in your/their household should be isolating (staying at home)?
    9) Has anyone in your/their household taken medication due to fever in the last 5 days?
    Did you answer yes to any one of the above questions?If you answered yes, you cannot attend the EarlyON program today. See below what you will need to do before attending another session.

    Next Steps (if you answered yes):

    1. ​You can not attend the session.
    2. You and your child(ren) should isolate (stay home) and not leave except to get a COVID-19 test or for a medical emergency.
    3. Talk with a doctor/health care provider to get advice or an assessment, including if they need a COVID-19 test.
    4. You may return once it has been 10 days since symptoms started and you/your child is feeling better.
    • If you think they have a cold or the flu, they should still talk with a doctor or get tested. Symptoms are similar to COVID-19.

    Notice of Risk:  When children from multiple families attend a single centre, there is an increased risk of the COVID-19 virus coming into the centre. Children who are infected with the COVID-19 virus are more likely than adults to have very mild infections or to have no symptoms at all, but these children can still transmit the infection to other children and to adults in centre. This means that children can bring home an infection acquired in the centre and put other persons at risk. This centre (Centrepointe Childcare Services) has a screening process to help detect infections when there are symptoms; however, this screening process will not detect infected children or adults who do not have symptoms at the time of screening. The risk of serious COVID-19 infection increases with age, which older childcare providers should consider in terms of the risk to themselves personally, particularly if working in childcare centres with children from multiple families.
